Membership Application

INSTRUCTIONS & IMPORTANT INFORMATION

Please print and read these instructions and information before completing application.

ALL applicants must:

• Complete PART ONE, being careful to check the appropriate membership category box.
• Sign and date the application on the line provided.

THERE IS A ONE-TIME INITIATION FEE OF $10 FOR ALL ADULT APPLICANTS. JUNIOR APPLICANT FEE IS $5.

Junior members are not liable for dues since they receive no member services apart from a certificate.

Adult dues are pro-rated based on date of application. If joining in:

• Sept., Oct., Nov. - dues are $30 plus $10 initiation fee: $40 total
• Dec., Jan., Feb. - dues are $22.50 plus $10 initiation fee: $32.50 total
• March, April, May - dues are $15.00 plus $10 initiation fee: $25.00 total
• June, July, Aug. - dues are $7.50 plus $10 initiation fee: $17.50 total

When making out your check or money order, PLEASE BE SURE TO INCLUDE $10 FOR INITIATION FEE AS WELL AS THE CORRECT AMOUNT OF DUES!

Please make your check or money payable to "Descendants of Mexican War Veterans" or "DMWV". No cash please!

FULL MEMBERSHIP applicants must also:

• Complete PART TWO and FOUR. Submit documentary proof of qualifying veteran ancestor's military service or civilian ancestor's employment. See below.

Acceptable Documentary Evidence of Veteran Ancestor's Service

Documentary evidence of an veteran ancestor's military service must provide the name and rank of the veteran and identify the military unit (i.e., company, regiment, battalion, ship, etc.) with which he served. A legible reproduction (i.e., photocopy) of any one or more of the following named documents are acceptable:

1. Discharge certificate
2. Muster roll page
3. National Archives Mexican War Volunteers index card
4. National Archives Regular Army, Navy, or Marines enlistments index page or card
5. Bounty land warrant
6. Approved pension application
7. National Archives Old Wars or Mexican War pension application index card with indication that pension was approved
8. Listing in Heitman's Roster of U.S. Army Officers, Robart's Mexican War Veterans, or a similar printed roster of U.S. military officers
9. Printed roster published by a state government
10. Printed roster transcribed from official muster rolls

Evidence of the service of a civilian employee of the United States who served in the War with Mexico shall consist of the legible reproduction of any document generated by the federal government or a state government that provides evidence of said service.

The following items (originals or reproductions) are NOT acceptable as evidence of service of any military veteran or civilian employee.

1. Pages from a printed Mexican War pension index
2. Obituaries
3. Family history books
4. Newspaper articles
5. Pages from printed rosters that have not been transcribed from original muster rolls
6. Photographs of grave markers

Substitutions

In the absence of any of the acceptable items listed above, any other document (such as a diary or letters) not specifically excluded may be accepted as evidence of service at the discretion of any person authorized to review said evidence and approve applications for membership.

Whenever a substituted document is submitted as evidence, it must be accompanied by a letter signed and dated by the applicant, stating that none of the acceptable items are available. Copies of many of these documents may be obtained from the National Archives in Washington, D.C.

IMPORTANT INFORMATION REGARDING PROOF OF SERVICE

• Send photocopies only. Please do not send original documents.
• Copies of pages from printed rosters are acceptable as proof only if the roster was compiled from original muster rolls (such as Charles Spurlins' Texas Veterans or Steven Butler's Alabama Volunteers in the Mexican War).
• It is not necessary to submit any other proofs! Sending any other documentary evidence is entirely at the discretion of the applicant. All materials sent will be placed in the applicant's permanent membership file.
• Copies of pages from Pension indexes are not acceptable proof since not all pension applications were approved. Note: The letter "A" preceding a pension application number in a printed index indicates the application was NOT approved. This means the applicant could not provide proof of service.

When you have completed this application, please send it by regular U.S. Mail (no certified or registered application packets please!), along with proof of veteran ancestor's service, to:
